Duties during this time included but were not limited to the following.
- Provided and arranged for training/instruction of Home Health Aids in the Central Ohio Office.
- Maintained accurate, detailed reports and records as appropriate for several payer sources such as Passport and several county agencies.
- Provided Q.A. supported Q.I. toward compliance with State regulations in Ohio.
- Presented in-services and training material monthly for all Home Health Aides.
- Performed administrative and managerial functions, such as taking responsibility for staffing, budget compliance, and long-range goals.
- Consulted and coordinated with Nursing and Hospice departments to assess, plan, implement and evaluate patient care plans.
- Assessed the needs of individuals and families to identify care needs and plan for delivery and documentation of that care.
- Successfully handled concerns brought forward by clients and family members.
- Instructed individuals, families and other groups on topics such as health education, disease prevention, and developed health improvement programs.
- Modified patient treatment plans as indicated by patients' responses and conditions.
Worked for the first five years as a Home Health Nurse Case Manager providing services to the frail elderly. Services included but not limited to the following.
- Wound Care, I.V. therapy, medication management and teaching.
- Supervision of Home Health Aides in the home setting.
- Writing, implementing and monitoring the Plan of Care consistent with the Physicians orders.
- Monitoring the patients for effectiveness of the Plan of Care and documenting all findings. All of the above performed within the parameters of Medicare and Nursing rules and law.
Accomplishments:
